Patient Care Experience
You can expect to receive expert care in a safe, nonjudgmental environment. The process is built around the needs of the patient, not algorithms or insurance plans. The approach is insight-oriented, practical, and interactive. Your care is based on your needs. Virtual visits are available after the first visit, which must be in person.
Initial consultation
Treatment begins with a 90 minute 1:1 evaluation, review of medical history, information from previous providers, and review of labs or imaging as indicated. After the initial evaluation, you will receive a preliminary diagnosis and treatment recommendations. Emphases are: ruling out general medical causes, integrating previous therapy experiences, assessing need and extent of therapy, and providing a foundation for exploring deeper more complex issues. My theoretical orientation is psychodynamic, and my technique is transference based and Socratic.

CBT techniques are supported but not specifically prescribed.

The style is direct, warm, but professional.

Psychopharmacology
Medication is not a substitute for change, but it can help to manage symptoms, especially in a crisis. Effectiveness, simplicity, and safety guide the practice. Alternative approaches into treatment can be integrated, but traditional allopathic medicine is the primary mode of treatment.

Medication management and psychotherapy should be provided together because it is the simplest and most effective design for the patient. For patients that already have therapists, medication management only is available.

Policies
Psychotherapy relies on a trusting, confidential, therapeutic environment. The following rules are based on the ethical guidelines provided by the American Psychiatric Association. Because psychotherapy is a unique experience, policies and expectations are stated clearly up front. Further elaboration is available at your request. Initiation of treatment will serve as acknowledgement of policies.

Boundaries
Once a patient, always a patient. After treatment begins, we cannot meet in any other capacity. Meetings occur only in the office or in the hospital. Family members are included only when the patient is present. Family can share clinically relevant information in writing. (except in an emergency)

Roles
As your psychiatrist, doctor and patient is the only relationship that is permissable.

Transparency
Pharmaceutical companies, hospital groups, or health care organizations are not involved in your care whatsoever. I do not hold patents on any products or procedures or have any financial ties with any other providers. You can expect transparency in clinical decision making and candor about effectiveness, risks, and benefits of specific treatments.

Cancellations
Patients will be expected to pay the full fee, if appointments are cancelled less than 24 hours ahead on NexHealth. IF THERE ARE UNAVOIDABLE CIRCUMSTANCES , THE FEE WILL BE WAIVED.

Insurance
I do not participate in any insurance plans. All communications regarding reimbursements and payment are between you and your insurance company. I will complete prior authorizations for medication and provide completed claim forms and super bills.

About Dr. Isaac
As a native of Washington D.C., I have been practicing in Northern Virginia and Washington D.C. for 22 years. Alongside of a private practice, I was a psychiatric hospitalist at INOVA Fairfax Hospital for ten years and a community psychiatrist in Washington D.C. for five years, I taught at Virginia Commonwealth University as an Assistant Professor, and currently supervise residents from Georgetown University School of Medicine and George Washington University School of Medicine. I was born in the United States, but my family is Eastern European.
Restructuring attachments, optimizing role functioning, interpretation of transference, and reframing developmental experiences through insight are the core features of my technique. The school is contemporary psychodynamic, with training in Brief Psychodynamic Psychotherapy and Cognitive Behavioral Therapy.

My approach to patient care is collaborative, egalitarian, flexible, non-judgmental, and transparent. As a patient, your ethnic, racial, political, religious, or partisan identities CANNOT adversely effect the delivery of your care. However, since these group memberships form "identity" they are often relevant.
